If this sounds like a service that you would like to use, please get in touch for more information. We’re happy to come and have a chat, do a Zoom call or talk over the phone to understand what you’re looking for as a manager to help you deal with requests for assessments in a timely and safe manner.

Briefly describe the communication provided: Communication has been consistently excellent—clear, professional, and timely. Responses are provided promptly, with relevant information communicated effectively and in a way that is easy to understand. There is also a strong sense of reliability and attentiveness, ensuring that queries are addressed quickly and nothing is left unclear or unresolved.

Did you find the service factual, transparent and open? If no – how can we do better? The service was factual, open, and completely honest. I always felt well informed. I genuinely can’t think of anything that could have been done better. I was extremely happy with the service I received and would have no hesitation in recommending it to others.

Did you feel that the service was for you and your Service User? In other words, did you feel in control and ‘heard?’ Yes, absolutely. I felt that the service was tailored to us and the Service User, and that our individual needs and circumstances were listened to and taken seriously. I always felt heard, involved, and kept informed, which gave me confidence that I was in control and that our views and wishes were respected throughout.

Did the Service User return home safely, appropriately and as described by the Trusted Assessor supporting you? Yes, the Service User returned home safely and appropriately, exactly as described by the Trusted Assessor. The arrangements were clearly explained to us beforehand, and everything was in place to support a smooth and safe return home. The information provided was accurate and reassuring, and the Service User’s needs were fully considered. The return home went as expected, with noissues or concerns, and we felt well supported throughout the process.

How did we do? You did extremely well. The service was excellent, prompt, and very professional throughout. Communication was timely and clear, and all reports and information about the Service User were very detailed, accurate, and helpful. I particularly appreciated the care taken to make sure the Service User was genuinely ready before discharge. You do not simply allow discharge to go ahead until you are satisfied that the appropriate arrangements and support are in place. This is especially reassuring because, in our experience, hospitals can sometimes be too keen to discharge people before they are fully ready or properly supported. Overall, we were extremely impressed with the service and felt that the Service User’s safety, wellbeing, and individual needs were always put first.

What can we do to improve the service? I honestly cannot think of anything that could be improved. The service has been excellent in every respect, and I have been extremely impressed with the quality, professionalism, communication, and support provided throughout.

Finally, can you write in your own words how you found the service and would you use it again? We have found this to be a very good and extremely valuable service. It enables us to support Service Users to return home safely with the correct care package and any necessary equipment in place. This has helped to prevent a significant number of failed discharges, which we experienced much more frequently in the past before using this service. Whenever one of our Service Users is admitted to hospital, or when we are asked to assess a package of care for someone who is in hospital, we always contact the Trusted Assessors. We have complete confidence in them because they consistently provide an excellent, reliable, and professional service. They take the time to fully understand the Service User’s needs and make sure that the appropriate care and support are in place before discharge. We would absolutely use the service again and would highly recommend it to others. It has made a real and positive difference to the quality and safety of our Service Users’ discharges home.

Testimonial left for Lauren and Mandy

The TA service has been an invaluable resource for us and has provided such valuable support when our clients have been admitted to RBH and PGH.

Being able to contact the team for assistance and guidance during what can often be a difficult and worrying time for our clients and their families has been hugely appreciated, so it will certainly be missed.

I did want to take the opportunity to say thank you to you and the team for all the support you have provided to us.

Debbie Emm – Registered Manager

We as a team at BCP council in house services-  Supported Living services would like to express our sincere and heartfelt thanks for all of the support you have provided as our trusted assessors service in the past.

Your support has been invaluable when our individuals we support have been admitted into hospital.

Your guidance, knowledge and co-operative approach have made a real difference, helping to ensure smoother transitions and better outcomes during what can often be challenging times.

We are naturally very disappointed that this service has now been withdrawn due to funding, as it has been such an essential resource for both the people we support and our team.

We truly hope that the funding will be reviewed in the near future and that this invaluable service can be reinstated.

In the meantime, we want to thank you and your team once again for everything you have done. We wish you all the very best for the future and hope our paths cross again soon.

The Trusted Assessors are truly invaluable within our system. Demand for their expertise has continued to rise, and their service is consistently and heavily relied upon across both UHD sites. They form a critical pillar of the entire discharge ecosystem, ensuring that patients move safely and efficiently through the pathway.

Their involvement prevents duplication of work, reduces delays, and significantly speeds up response times. They are consistently responsive, thorough, and highly expert, bringing a level of clinical insight and practical judgement that is essential for safe, timely discharge planning.

For our most complex patients, they are absolutely the right people to be involved. Their assessments are robust, their decision‑making is sound, and their ability to navigate complexity is unmatched. As the discharge lead, I rely heavily on their experience, their knowledge, and their professional confidence. The depth of their skill set is something I could not replicate myself, and their contribution directly strengthens the quality and safety of our discharge processes.

Quite simply, the Trusted Assessors are an indispensable part of our service, and the system would not function to its current standard without them.

A massive shout out to Lou for helping me with a lady with complex needs recently, it was a terrific assessment .
